Job Description
The Population Health Care Management RN complements the practitioner-patient relationship through support of the established plan of care, using cost-effective, recommended practice guidelines in support of the HIV continuum. The goal is to address acute needs, prevent or delay severe stages of disease progression, and enhance the client's quality of life. This position reduces complications and morbidities to improve health and reduce healthcare costs. The RN coordinates, educates, and provides expertise to clients living with HIV/AIDS across the continuum of care from complex medical to chronic conditions and promotes compliance with preventive care measures. Working as part of a multi-disciplinary team, the RN coordinates efforts to identify clients with the highest levels of morbidity, risk, lack of adherence, or barriers to care, and implements plans to address these needs.
