Job Summary
Guideway Care is looking for a subject matter expert in Medicare’s Chronic Care Management (CCM) program to oversee the training and support of the CCM business. The Chronic Care Manager will be responsible for promoting quality patient engagement and ongoing customer support through the effective leadership of the clinical staff and implementing strategic plans, policies, and procedures related to the CCM business.

Job Summary
Guideway Care is looking for a subject matter expert in Medicare’s Chronic Care Management (CCM) program to oversee the training and support of the CCM business. The Chronic Care Manager will be responsible for promoting quality patient engagement and ongoing customer support through the effective leadership of the clinical staff and implementing strategic plans, policies, and procedures related to the CCM business.
Responsibilities:
- Stay abreast of all CMS regulations for CCM, RPM, and TCM services
- Hire, coach, train, and manage the call quality of clinical staff personnel to maintain high customer service standards
- Conduct effective resource planning to maximize the productivity of resources (people, technology etc.)
- Distribute patients appropriately and review, manage, and report the daily production of clinical staff
- Manage client reporting for CCM/RPM/TCM clients
- Develop outreach protocols for clinical staff that ensure compliance with regulationsand excellent patient experience
- Minimum of 3 years experience with Medicare’s Chronic Care Management program
- Experience working in a call/contact center setting
- Experience working with Electronic Health Records (EHR)
- Demonstrated customer service, leadership, communication, interpersonal and problem-solving skills, required
