Borinquen Medical Centers is based in Miami, Florida and is seeking to hire a full-time Community Health Worker (CHW) to join our team. The Community Health Workers (CHW) will provide culturally competent and effective education and connection to services to our Geriatric Population, specifically on individuals who have chronic conditions such as hypertension, diabetes, asthma and obesity. Works with clinical staff to ensure efficiency and productivity.

Tasks And Responsibilities

  • Manages all Geriatric patient services across all centers to ensure uniform policy implementation
  • Consistently pulls and audits reports to ensure there are no flow gaps
  • Compiles reports of flow gaps, providing counseling to other staff as needed
  • Performs and oversees the following departmental tasks:
  • Services may require to be offered during both traditional and non-traditional work hours
  • Maintain a log of activities and complete regular reports to meet grant objectives. Activity log will be prepared daily to measure productivity. In addition, monthly reports will be prepared and submitted on the last working day of each month.
  • Document and report result of outreach encounters within eight (8) hours after each encounter. Example: referral source, age, race, sex, risk factors… attempts to contact via phone calls, letters, and missed appointments.
  • Coordinate patient appointments referrals to mitigate SDOH and improve linkage to care.
  • Attend community food distribution, testing and vaccination events to provide outreach and support
  • Assist clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services.
  • Build and maintain relationships with community members and organizational partners
  • Follow-up with health management/care plans with both patients and providers
  • Attend all staff meetings, in-service training, and periodic seminars to enhance knowledge on Outreach education when schedule.
  • Adhere to all of the agency’s policies and procedures.
  • Work with other relevant community-based organizations in the project.
  • Works with partnering organizations and community events to recruit new patients
  • Works with billing and insurance verification to insure maximum revenue potential
  • Interpret and explain information such as eligibility requirement, application details, payment methods, and applicants’ legal rights
  • Other duties as assigned by immediate supervisor

Requirements

  • High School Diploma (some college preferred).
  • Any combination of 3 years health/social services experiences and /or education.
  • Verifiable good driving record and reliable transportation.
  • Bilingual/Trilingual preferred ; Creole/Spanish
  • Strong interpersonal skills.
  • Ability to interact effectively with consumers, administration, faculty, and staff.
  • Handle information with high level of confidentiality.
  • Considerable knowledge of functions of Social Services that involve working with, communicating with, and teaching people. These occupations often involve helping or providing service to others.
