Medical Social Worker - Home Health

We are currently seeking a passionate and dedicated Per Diem Medical Social Worker (MSW) to provide support and care in the comfort of patients' homes.

Location: Bend, OR

Salary: Starting at $38/ hour, DOE!

Qualifications:

• Graduate of a Master’s program in social work accredited by the Council on Social Work Education
• Minimum of one (1) year of experience in healthcare, with preference given to candidates with experience in home health care
• Excellent verbal and written communication skills, as well as strong organizational abilities
• Current CPR certification
• Must be a licensed driver with access to a reliable, insured vehicle in good working order
  
  
  

Responsibilities:

• Assess the psychosocial status of patients in relation to their illness and environment
• Conduct social evaluations and develop intervention plans based on assessment findings
• Maintain accurate clinical records for all patients referred to social work services
• Provide information and referral services to patients and families/caregivers regarding practical and environmental needs
• Serve as a liaison between patients/families/caregivers and community agencies
