Job Description

The Registered Nurse is responsible for collaborating with services internal and external to the VA to facilitate care transition to effectively meet the veterans care needs. The RN demonstrates leadership by serving as an advocate for patients, and a team leader with colleagues as she/he continues to enhance his/her own and the team's professional growth, development, and practice.

Key Duties And Responsibilities

  • Demonstrates leadership and mastery of practice in delivering and improving efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication, and coordination of resources
  • Executes position responsibilities that demonstrates leadership, experience, and creative approaches to management of complex client care
  • Demonstrates performance and leadership that is broad enough to improve the care for a group of patients
  • Collaborates with patients to assess and identify needs, issues, care goals, and resources in critical factors for achieving desired outcomes for discharge, post hospital recovery and health maintenance/wellness
  • Advocates fiscal responsibility through monitoring utilization of resources
  • Enhances patient self-management by providing information regarding all options, choices, and resources
  • Actively participates in care delivery with priority attention to patient preference
  • Promotes cost effective use of resources; effective least-expensive strategies expected in non-face-to-face approaches (including telehealth)
  • Develops options with patients and families, including costs, alternatives, risks/benefits and services and monitors the provision and outcome of care provided
  • Maintains privacy and confidentiality of all patient information, including electronic, print, and conversations
  • Attention to detail in all aspects of the patient's medical record and to ensure that all overdue preventative measures are up to date

Knowledge & Skills

  • Utilizes judgement in selecting the appropriate order and sequence of procedures and treatments.
  • Utilizes judgment in selecting the most appropriate application of procedures and guidelines.
  • Initiates and participates in preventative health screening and instructions for ambulatory care patients.
  • Observes, identifies, and responds to patients' needs for care, including medication and equipment-assisted care, and recognizes deviations from normal.
  • Performs venipuncture and immunizations upon completion of competency where indicated
  • Ability to perform and maintain vitals, review of systems, and other information pertinent to quality care for the patient
  • Working within multiple technology platforms, to include an Electronic Medical Record System, with ease and comfort
  • Experience with case management and providing educational resources tools to patients regarding their specific treatment plan
  • Ability to work with complex patients and providers to distinguish the best treatment plan for a patient
  • Problem solve with the ability to de-escalate situations or patients

Required Qualifications

  • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education or Training: Graduate of a school of practical or vocational nursing approved by the appropriate State Agency and/or accredited by the Accreditation Commission for Education in Nursing (ACEN); OR Received health care education/training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualified for full RN licensure.
  • Licensure: Full, active, current, and unrestricted licensure as a registered nurse in the State of Texas
