Summary

  • The Social Worker – Disease Management provides services to TRICARE beneficiaries, working closely with the clinical team to assess, plan, implement, coordinate, monitor, and evaluate the beneficiary’s health and psychosocial needs.
  • This role emphasizes empowering beneficiaries to manage their chronic conditions effectively, preventing complications, and minimizing risk through evidence-based tools and support.
  • Key responsibilities include contributing to care planning, educating beneficiaries, managing resource utilization, and facilitating transitions between care regions.

Key Responsibilities

  • Assess beneficiaries' medical, physical, and psychosocial needs to support their overall goals of care.
  • Collaborate with the clinical team to create comprehensive care plans, integrating behavioral and psychosocial assessments.
  • Provide education and coaching to manage chronic conditions effectively.
  • Ensure appropriate use and access to medical and behavioral resources, maintaining high standards of care.
  • Identify and address health disparities and facilitate resource access through SDOH assessments.
  • Develop measurable outcomes to monitor the impact of programs and services on beneficiaries' health.
  • Assess risk factors and need for additional TRICARE services and resources.
  • Facilitate beneficiary transfers between regions, coordinating with military liaisons for smooth transitions.
  • Adhere to URAC standards, TRICARE policies, and nationally recognized practice standards.

Working Conditions

  • Ability to cover various work shifts and work overtime as needed.
  • Onsite work in a standard office environment or remote work with a secure, high-speed internet connection.
  • Extensive computer use, including prolonged sitting, headset use, typing, and phone communication.

Education & Experience Required

  • Current, unrestricted Social Work licensure (BSW) in the state of residence.
  • U.S. Citizenship.
  • Ability to obtain a favorable Interim and final Department of Defense (DoD) background investigation.
  • Minimum of two years’ experience in case management, care coordination, or discharge planning in healthcare or managed care settings.

Preferred

  • Experience in care or disease management with a managed care organization (MCO).
  • Master’s degree in Social Work (MSW) and LCSW or LMSW licensure.
  • Knowledge of government healthcare programs, including TRICARE and Medicare/Medicaid.
