Nurse Director of Case Management

Position: Nurse Director of Case Management
Location: Ventura County, CA

Duration: 12+ Months

Job Responsibilities:

• Develops, manages, and oversees the annual care management budget. Prepares and evaluates monthly, quarterly, and annual reports of the department's functions. Monitors and reports on changes in Medicare regulations and documentation issues to physicians and others as needed. Maintains relationships and contractual oversight with key stakeholders, local, state, and federal agencies.
• Oversee a team of 25, case managers and social workers.
• Licensed RN with the BSN, Masters preferred.
• 5+ years hospital discharge planning, utilization review, case management and social work experience, with management experience.
• RN-BC Certification in Care Management.
• No job hoppers, must explain any gaps in employment.
• Must be a current Manager or Director CM for this role.
