Key Responsibilities:

Patient Engagement:

  • Proactively contact patients in the RPM diabetes program to check on their status and adherence to monitoring plans
  • Provide motivation and encouragement to enhance patient engagement with health coaches and care plans
  • Build and maintain strong relationships with patients to foster trust and continued participation

Educational Support:

  • Provide educational resources to help patients understand and manage their diabetes more effectively
  • Conduct one-on-one or group sessions to reinforce the importance of adhering to the monitoring protocols within the care plan
  • Act as a key resource for patients, offering guidance on diabetes management and support

Feedback Collection and Analysis:

  • Gather and analyze patient feedback to understand their needs, concerns, and satisfaction levels
  • Use patient feedback to enhance engagement strategies and improve the effectiveness of the RPM program

Collaboration:

  • Work closely with healthcare teams, including diabetes health coaches and clinical staff, to ensure patient care plans are communicated effectively and any issues are promptly addressed
  • Collaborate with other departments to ensure a seamless and supportive experience for RPM patients

Reporting:

  • Maintain accurate and detailed records of all patient interactions and outcomes in patient management systems

Qualifications:

Education:

Degree in healthcare management, nursing, social work, or a related field. Additional training or certifications in diabetes management is a plus.

Experience:

Minimum of 2-3 years in patient retention, healthcare customer service, or case management, with a focus on chronic disease management preferred.

Skills:

  • Strong communication and interpersonal skills
  • Proficiency with patient management systems and CRM tools
  • Excellent problem-solving abilities with a creative approach to patient engagement
