- Act as a key resource for patients, offering guidance on diabetes management and support
- Gather and analyze patient feedback to understand their needs, concerns, and satisfaction levels
- Use patient feedback to enhance engagement strategies and improve the effectiveness of the RPM program
- Work closely with healthcare teams, including diabetes health coaches and clinical staff, to ensure patient care plans are communicated effectively and any issues are promptly addressed
- Collaborate with other departments to ensure a seamless and supportive experience for RPM patients
- Maintain accurate and detailed records of all patient interactions and outcomes in patient management systems
- Provide regular reports on patient retention metrics, challenges, and successes to relevant stakeholders
Education:
