Covenant Case Management Services is seeking an energetic and compassionate Qualified Professional for Individual and Family Directed Servicesto join our team in providing Individual and Family Directed Services. This role is vital in leading a dedicated team that supports individuals with intellectual and developmental disabilities (I/DD) while ensuring high-quality service delivery.

Major Responsibilities

  • Team Leadership: Develop, supervise, and coordinate direct care staff and contractors to ensure effective service delivery
  • Staffing Oversight: Manage staffing schedules and coordinate backup staffing as necessary
  • Supervision and Guidance: Provide ongoing supervision and support to I/DD employees/contractors, focusing on quality service provision
  • Collaboration: Establish and maintain positive working relationships with Managed Care Organizations (MCOs) and other partner agencies
  • Person-Centered Assessments: Conduct assessments using a person-centered approach, developing measurable short-term goals aligned with long-term treatment plans
  • Training: Train support staff and contractors on individual care needs, including Individual Support Plans (ISPs)
  • Authorization Management: Ensure timely submission of service authorizations to maintain ongoing services
  • Progress Reporting: Complete quarterly progress reports and participate in internal audits as required
  • Advocacy: Advocate for individuals and families to ensure necessary services are provided
  • Monitoring and Compliance: Monitor services provided and ensure compliance with company policies, performance improvement measures, and HR requirements
  • Crisis Management: Be available on-call for crisis services during evenings and weekends as needed
  • Documentation Review: Review incident reports, progress notes, and billing data to ensure accuracy and compliance
  • Continuous Improvement: Implement and monitor standard operating procedures related to billing, documentation, and training

Skills & Qualifications

  • Communication Skills: Excellent verbal and written communication skills, enabling effective interaction with diverse stakeholders, including individuals served, employees, and external agencies
  • Leadership Abilities: Strong leadership skills with the ability to foster a positive working atmosphere that encourages growth and learning
  • Adaptability: Capability to perform various tasks efficiently and maintain composure in changing situations
  • Conflict Resolution: Skilled in negotiating and resolving conflicts between parties
  • Knowledge Base: Familiarity with behavioral sciences and disciplines related to the care and treatment of individuals with intellectual and developmental disabilities, mental health, and co-occurring disorders
  • Physical Requirements: Ability to lift up to fifty pounds

Equipment Requirements

  • Reliable access to the internet and a cell phone
  • Valid driver’s license and reliable transportation (car or truck)

Credentials, Certifications & Experience

Minimum Qualifications:

  • Bachelor's degree in a human service field, with:
    • Two years of full-time post-bachelor's degree experience in a relevant field, or
    • Four years of full-time post-bachelor's degree experience in a different field with relevant experience in mental health, developmental disabilities, or substance abuse counseling
Supervisory Experience: Minimum of 2 years.

Management Experience: Minimum of 2 years.
