Job Description

The Network Performance Specialist role will work for the Social Care Network responsible for the Southern Tier region of New York and will be responsible for managing successful participation and performance of nutrition services by participating organizations. As a local subject matter expert in this field, this role will help identify best practices, promote workflow enhancements, and host stakeholder communications with organizations across a large region of upstate New York. This position reports to the Network Operations Manager.

Essential Functions:

  • Serves as a regional expert of the new 1115 waiver health related social need nutrition enhanced services, working to ensure eligble services are delivered in a timely manner across the Southern Tier region, and provides recommendations to the department manager that improve enhanced service delivery across a network of community based organizations (CBOs).
  • Maintains enhanced knowledge of the HRSN nutrition services in the Southern Tier region and develops and deploys supportive materials to support hiring, onboarding, and training to support individuals involved in providing HRSN nutrition services in the Southern Tier.
  • Can seamlessly utilize the Care Compass Unite Us system in support of nutrition related service delivery, providing recommendations for system enhancements.
  • For organizations who maintain systems that integrate with Unite Us, this position will work to help ensure workflows and related information is properly recorded in the system.
  • Maintains high understanding of NYS DOH HRSN nutrition service requirements. As needed supports the development of communications to ensure network participants are aware of requirements or leading practices.
  • Directly support local CBOs to build capacity with HRSN nutrition capabilities. Leverage local and national CBO expertise to share best practices for HRSN nutrition services.
  • Regularly coordinate and collect feedback with network staff who perform HRSN nutrition services, garnering input on the salient challenges and barriers they are facing and collaborate around productive solutions. Create and execute timely action plans for identified gaps or deficiencies.
  • Provide tailored support to each CBO to ensure they not only meet but are able to maintain the defined readiness criteria, including validating responsible CBO staff have completed the required trainings.
  • Maintains a strong understanding of existing nutritional supports, services, and offerings. Successfully integrates and synergizes these existing services with the new 1115 waiver housing enhanced services.
  • When needed, works with the Network Operations Manager to develop performance improvement plans (PIP) for CBOs and providing supplementary training and technical assistance to help the CBO improve over time. As needed, develop corrective action plans to address areas requiring improvement.
  • Responsible for managing social care network performance in the focus area of HRSN nutrition including but not limited to monthly reviews of performance data, reviewing services provided against outstanding referrals to identify gaps, and challenges with access to HRSN nutrition services. Develop corrective action plans, where necessary, to address nutrition service deficiencies.
  • Provide performance reports (scorecards) to organizations providing HRSN nutrition services in the social care network, utilizing data analysis techniques to convey opportunities for remediation and influence needed workflow changes. As necessary, facilitate training on network/CBO performance improvement and performance monitoring. Track and communicate partner performance to internal and external stakeholders to ensure the social care network is meeting expectations.
  • Integrate continuous improvement in program design, execution and monitoring.

MINIMUM REQUIREMENTS:

  • Education: Bachelor s degree required and at least 3-5 years experience managing projects.
  • Experience with either screening, navigation, or case management activities, ideally with low-income or Medicaid individuals.
  • Ability to work in a high pace environment and with the ambiguity & change that comes with a new program.
  • Consistently utilizes critical thinking skills to resolve issues/challenges in workflows, generate process maps, and manages program performance.
  • Ability to convey complex or technical information in a manner that others can understand, and ability to understand and interpret complex information from others.
  • Results-driven and client-focused
  • Skilled with personal computer programs such as Microsoft Outlook, Microsoft Office (Word, Excel PowerPoint, etc.)
  • Facilitation of meetings either in person or via virtual platforms such as Zoom.

WORK ENVIRONMENT:

Care Compass Network utilizes a hybrid work model where employees are able to work remotely from a home office or from the CCN office, a non-clinical professional office setting. There may be some travel required for partner and/or network meetings.
