Job Overview

The Health Equity Program Manager is a subject matter expert that utilizes external and internal resources to promote advances within diversity, health equity, and inclusion in alignment with the Children’s Hospital Colorado mission, vision and values with a focus on patients and community-based health outcomes. This position leads both internal and external work groups and provides training and resources to build practices and procedures that encourage a culture of inclusion and address health disparities. The Health Equity Program Manager is also responsible for monitoring and measuring effectiveness of assigned diversity, health equity outcomes and inclusion programs

This role requires someone with proven skills in successfully launching and marketing a program.

Department Name: HP Community Health

Job Status: Full time, 40 hours per week

Shift: Most shifts between 8am and 6pm. Shifts of 8 to 10 hours per day. Some weekend and holiday program coverage will be required.Hybrid.

This position connects across a multitude of departments and divisions in the CHCO system, and will require some travel and connection in the broad CHCO community.

Qualifications

  • EDUCATION – Bachelor’s Degree in Communications, Education, Social Sciences, Public Administration, Management, Public Health Administration, Health Care Management, Human Resources, Social Work, or a related field.
  • EXPERIENCE – Five years of experience developing and implementing diversity, equity, and inclusion initiatives and/or public health programs.
  • EQUIVALENCY – Professional experience in a related field may substitute for the bachelor’s degree requirement on a year for year basis.

Essential Functions

An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.

  • Oversees the development, design and implementation of health equity programs based on the community health needs assessment and implementation plans, including activities designed to improve health, safety, family education/empowerment, and prevention. Delivers and provides training/outreach to internal and external partners.
  • Acts as a technical expert and spokesperson for the program, providing consultative and technical guidance to staff and providers; interacting with internal and external interest groups to explain program position, mediate differences, and seek modifications; and handles sensitive or controversial issues, providing resolution to problems.
  • Participates in development of program specific strategic plan, program annual goals, and performance metrics related to diversity, health equity outcomes and inclusion efforts; evaluates and monitors metrics to ensure goals and objectives are being met and communicates outcomes.
  • Manages the overall risk of the program, which includes conducting risk and cost/benefit analyses and devising strategies to deal with unexpected crises and unresolved risks
  • Develops and coordinates the community-based network that focuses on, and addresses health disparities identified by the Child Health Community Needs Assessment. Utilizes data to continually assess the efficacy of the community-based network and partner organization relationships.
  • Builds and manages partnerships with external organizations to support diversity, health equity outcomes and community health efforts. Utilizes internal networks and resources to enhance the culture of diversity, health equity and inclusion within the workforce.
  • Coordinates organization-wide and community events and programs which promote diversity, health equity and inclusion.
  • Utilizes data and analytics to pinpoint health and social disparities to enhance outcomes for patients, team members, and the community.
  • Serves as liaison between program, department, CHCO and community partners; develops and maintains program policies, procedures, guidelines, and standards.
  • May supervise assigned program team members or contractors to include interviewing, selecting, training and developing team members; assign and reviewing work; and defining performance standards and completing performance evaluations.

Salary Information

Annual Salary Range (Based on 40 hours worked per week): $76,368.20 to $114,552.29

Hourly Salary Range: $36.72 to $55.07
