Summary: Life Is Work is seeking an experienced and compassionate (Bi-Lingual) Non-Medical Case
Manager within our organization. The Non-Medical Case Manager is a critical role within our
organization, responsible for providing comprehensive case management services to clients. This
position requires a compassionate and organized individual who can effectively coordinate and
deliver support services to meet clients' needs, empowering them to achieve their goals and
improve their quality of life.
Key Responsibilities:
• Conduct thorough assessments to identify clients' needs, strengths, and goals.
• Develop individualized service plans in collaboration with clients, outlining specific
objectives and timelines.
• Complete all necessary intake documentation and maintain accurate client records.
• Connect clients with appropriate services and resources, both within the organization and
through external partners.
• Coordinate and facilitate access to healthcare, housing, education, employment, and other
supportive services.
• Monitor and follow up on clients' progress, adjusting service plans as needed to ensure
successful outcomes.
• Respond to client crises and provide immediate support and intervention.
• Develop and implement crisis management plans to address urgent needs and stabilize
situations.
• Coordinate with emergency services and other agencies as necessary.
• Collaborate with program managers, community partners, and stakeholders (internal and
external) to streamline service delivery and enhance client outcomes.
• Maintain comprehensive and confidential client records, ensuring compliance with
organizational policies and regulatory requirements.
• Participate in program development initiatives.
• Promote a culture of empathy, respect, and empowerment within the team and across the
organization.

Qualifications:
• Bachelor’s degree in social work, Psychology, Human Services, or a related field is
preferred.
- Master’s degree or relevant certification (e.g., LMSW, LCSW) is preferred.
• Bilingual (Spanish) is preferred.
• Minimum of 2-3 years of experience in case management, social work, or a related field,
preferably within a nonprofit organization. Strong understanding of social service
systems, community resources, and best practices in client-centered care.
• Excellent communication and interpersonal skills, with the ability to build rapport and
trust with clients, staff, and stakeholders.
• Ability to make sound decisions under pressure, prioritize tasks, and manage multiple
responsibilities effectively.
• Knowledge of data management systems and proficiency in Microsoft Office Suite.

Working Conditions:
• Full-time position may include occasional evenings/weekends based on program
needs/requitements.
Salary and Benefits:
• $45K – 50K Annually
