Your Specific Responsibilities

This is an independent field worker position in the control and prevention of Tuberculosis. Activities reflect knowledge and application of sound epidemiological surveillance, containment, and assessment principles. Assists in the performance of disease control and prevention program activities. Principal duties and responsibilities are as follows:

Direct Observe Therapy – Meets with the client at home, clinic, worksite, school, under the bridge, any other acceptable location to provide Direct Observe Therapy (DOT) to the Tuberculosis clients. Recognizes and assesses client for adverse reactions of medication and reporting any adverse reaction to the Nurse Case Manager (NCM). Provides the bottle or packages of medication to the client to take as prescribed. Observe the client ingest and swallow the medication. Enters services and tracks DOT/VDOT provided to client in HMS/Medication tracking module. Identifies Medical, social services, or personal need that could interfere with the successful completion of the treatment and reports findings to the case management team. Reports any failure of DOT or incident out of the ordinary to the NCM and document the incident in clients’ s HMS/progress notes. Ensures the DOT counseling form is reviewed and signed by the client. Provides appropriate language and or culturally sensitive DOT services for client with language and/or culture barriers.

Contact Investigation – Assists and/or conducts epidemiological investigation to identify contacts, assess risk factors, locate, test and refer them as appropriate. Enters and keeps update information in HMS/ Contact investigation module (documentations). Assist in achieving and keeping the contact investigation indicator at the goal level.

Pharmacy and Case Management – Assists in the success of the client’s case management plan by reinforcing TB education and encourage compliance with treatment: Collecting specimen (such as sputum, etc…); providing TST; requesting transportation; delivering incentives and/or enablers. Meets regularly with NCM to provide updates on client’s adherence. Obtains medication from pharmacy room and checks to determine the medication matches the medication orders and that the medication is given to correct client. Follows security procedures to keep medication safe for DOT performed in the field. Maintains client confidentiality following HIPAA laws and regulations.

Locate and Refer – Conducts field investigation to locate and refer client’s that are lost to follow and/or noncompliant. Documents findings through HMS/Progress notes.

Attends staff meetings, case management team meeting, case reviews, cohort review, TB consortium meeting, educational meetings, and conferences as needed. Enters client services (CSR), completes EAR, reads E-mail daily, completes timesheet and reviews follow up assignments in HMS/Clinical Portal.

Performs Other Related Duties As Required.

Required Knowledge, Skills, and Abilities:

Knowledge of methods of compiling, organizing, and analyzing data; skills in conducting fact finding interviews; Ability to prepare confidential records and reports; ability to establish and maintain effective working relationships with others in a stressful environment: ability to communicate effectively orally and in writing.

Qualifications

Minimum –

Valid driver’s license and access to a reliable vehicle to use for work purposes, experience working with a disease control program, social services program, or community-based organization.

Preferred –

The incumbent in this position is preferred to be bilingual in English and Spanish or English and Creole.
