We are seeking a Continuous Quality Improvement Coordinator/Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating and managing patient care to ensure appropriate services are provided. Responsibilities:

  • Conduct diagnostic evaluations to assess patient needs and develop individualized care plans
  • Collaborate with healthcare providers
  • Monitor patient progress and adjust care plans as needed
  • Maintain accurate medical records and documentation
  • Work with diverse patient populations including geriatrics and individuals with complex medical conditions
  • Complete chart audits for compliance
  • Educate staff for the purpose of improving compliance

Qualifications:

  • Familiarity with medical terminology and procedures
  • Strong communication and interpersonal skills
  • Ability to work effectively in a multidisciplinary team environment
  • Experience working with electronic health records systems

Requirements:

  • Previous experience as an ADvantage case manager not required
  • Licensed as an LPN or RN OR Bachelor's degree required
  • One year working with elderly adults required
