We are seeking a Continuous Quality Improvement Coordinator/Case Manager to join our healthcare team. Responsibilities:
- Conduct diagnostic evaluations to assess patient needs and develop individualized care plans
- Collaborate with healthcare providers
- Monitor patient progress and adjust care plans as needed
- Maintain accurate medical records and documentation
- Work with diverse patient populations including geriatrics and individuals with complex medical conditions
- Complete chart audits for compliance
- Educate staff for the purpose of improving compliance
- Familiarity with medical terminology and procedures
- Strong communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team environment
- Experience working with electronic health records systems
- Previous experience as an ADvantage case manager not required
- Licensed as an LPN or RN OR Bachelor's degree required
- One year working with elderly adults required
