Illumination Foundation (IF) is a growing non-profit organization dedicated to disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services, and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County, and the Inland Empire.

Job Description

The Medical Intake Specialist plays a key role in supporting the our client care initiatives by conducting intakes at Illumination Foundation Facilities, assessing medical and mental health status, and coordinating necessary care services. This position involves close collaboration with medical coordination assistants to ensure comprehensive support for clients' needs, particularly post-hospital discharge.

The pay range for this position is $21.50-$24.00 per hour, depending on experience.

Responsibilities

  • Perform initial intakes to ensure proper client discharge from hospital with all appropriate needs for the continuation of care and documenting all findings
  • Assess medical and mental health status of client upon arrival by conducting head to toe assessment, checking vitals, determining insurance status, making follow up appointments with clients’ doctors, and picking up/filling prescriptions as needed
  • Maintain internal agency documentation requirements in agency EHR and Microsoft Office
  • Utilize the Homeless Management Information System (HMIS) to enter client information, if needed
  • Demonstrate a strong understanding of CalOptima and Medicare insurance and utilize this knowledge to refer clients to insurance funded programs that assist with social determinants of health
  • Orient clients to facility amenities and rules upon intake
  • Assist medical coordination assistants with client service connections as needed
  • Assess client need for recuperative care services
  • Acts in an ethical manner reflecting core values of integrity, transparency, accountability, respect and responsibility. Acts above reproach in all that is done on behalf of Illumination Foundation
  • Models’ openness, honesty and accountability to colleagues, volunteers, donors and all others involved with Illumination Foundation
  • Must accept personal responsibility and accountability for performance of duties
  • Must maintain confidentiality of work-related information and materials

Preferred Experience/Minimum Qualifications

  • Valid CA Driver’s License and eligibility for company vehicle insurance
  • High School Diploma or equivalent
  • Customer and client relations skills
  • Proficiency in Microsoft 365
  • Understanding of Cal Optima, Medicare, and continuum of care homeless services
  • Medical Assistant certificate or Emergency Medical Technician Certificate

Preferred:

  • 2+ years working in homeless and/or geriatric services preferred
  • Bilingual in English and Spanish
