Objective

The Cancer Support Coordinator (CSC) is dedicated to providing high-quality support to cancer patients and caregivers by matching them with trained volunteer peer mentors based on specific criteria and individual needs. This role also includes supporting and maintaining CHN’s partnerships with healthcare institutions, fostering a strong mentor network, and helping grow CHN's reach within the healthcare community.

Qualifications

  • Patient navigation, social work, mental health, or other healthcare-related experience required (minimum 3 years)
  • Empathy, compassion, and the ability to handle sensitive and emotional situations with professionalism
  • Knowledge of oncology care, treatment procedures, and support services is highly desirable
  • Ability to work independently, take initiative, and solve problems
  • Experience with computer data entry and retrieval and ability to utilize supplementary software programs and online applications (e.g.: MS Office, email marketing, blog platform, etc.)
  • Bilingual English/Spanish fluency preferred

Key Responsibilities

  • Mentor Matching & Coordination: Implement and manage the peer mentor matching process for cancer patients and caregivers, ensuring alignment with the organization’s operational metrics and unique needs of each client. Conduct accurate intake, entry, and assignment of mentees, with ongoing follow-up communication.
  • Volunteer Management & Training: Support the management of CHN’s volunteer network by offering ongoing training, communication, and assistance to Peer Mentors. Ensure that volunteers have the resources they need and that mentee-mentor matches run smoothly.
  • Database & Reporting Maintenance: Maintain and update CHN’s matching database with accurate and timely information. Report on program impact by preparing quarterly reports highlighting program progress and key outcomes.
  • Mentee & Mentor Support: Provide compassionate support to mentees and mentors, addressing any questions or concerns and connecting clients with additional vetted cancer support organizations as needed.
  • Program Promotion: Support CHN’s efforts to promote peer support services by coordinating marketing activities in partner institutions, including informational materials for patients (e.g., flyers, digital screens, patient packets). Collaborate with partners to increase visibility and awareness of CHN services.
  • Event & Meeting Participation: Assist in planning and attending CHN volunteer recognition events, as well as participating in CHN fundraising and external partner events.
  • Team Collaboration: Collaborate with the Director of Programs & Advocacy, Director of Partnerships & Impact Initiatives, and support the Client Services Lead, as necessary.

This position is remote, part-time; 20 hours per week, hours flexible
