Overview

Florida Assertive Community Treatment (FACT) is a dynamic multidisciplinary treatment team dedicated to deliver intensive community-based support to the severely mentally ill adult population. The purpose of FACT Peer Specialist is to be a part of the multidisciplinary treatment team designed to help individuals maintain stable functioning in the community by promoting hope and strong advocacy and empowerment of client’s self-directed goals. The FACT Peer Specialist uses his or her lived experience to serve as a mentor in providing support for persons with severe mental illness and/or substance use disorder. The role is non-clinical and does not include treatment, assessment and/or clinical evaluations of clients. It involves developing a trusting relationship to build clients’ resilience, hope, recovery skills, and connects them to treatment services.

Essential/Core Job-specific Duties And Responsibilities

  • Individualized Treatment Team (ITT): Attends the client’s ITT meetings and provides essential expertise to the entire team to promote a culture in which each person’s point of view and preferences are recognized, understood, respected, and integrated into care. Assist clients in communicating personal goals with their case management and other clients/prescribers in determine the steps to take to achieve goals.
  • Wellness Recovery Action Plan (WRAP): Assist client in developing WRAP in order to help clients, discover safe wellness tools and create a crisis/post-crisis plan. Accomplish through regular meetings to help clients identify their own strengths and support them in using those strengths to communicate goals to staff and clinicians.
  • Service Delivery: Conducts regular meeting with clients on an individual group and family basis in the office and in community settings. Core services include:
  • Role model competency in recovery, resiliency, wellness strategies and other techniques to the clients. Share own personal recovery journey story and the value of recovery to teach clients appropriate self-advocacy to help clients sustain recovery success.
  • Advocate strengths and needs of clients to family members, other natural support, staff and other clinicians.
  • Assist clients in working with their case manager and other clinician/prescriber in determining the steps to take to achieve the personal goals and self-directed recovery goals through individual and/or group setting. Support clients in finding and developing the recovery goals by providing skill building and resources to aid them in order to achieve those goals.
  • Advocates for clients at appointments and services within the healthcare systems.
  • Progress Notes: Documents progress of persons served to maintain a permanent record of individual activity per established methods and procedures. Documentation must be finalized in Avatar within 48 hours from the time the services were rendered. Progress notes must be detailed and clearly reflect how the staff’s efforts are linked to the services and goals in the client’s service plan and the client’s progress or lack of progress relative to the service plan.

Hours will vary and rate of pay is $15.00 per hour (non-certified) and will increase to $17.00 per hour when you become certified. Apalachee Center will be available to assist you with gaining certification.
