  1. Make sure the webpage is open.
  2. In the Safari app on your Mac, choose Safari > Preferences, then click Security.
  3. Make sure the Enable JavaScript checkbox is selected.
  4. Click Websites.
  5. On the left, click Content Blockers.
  6. Make sure Off is chosen in the pop-up menu next to the website.
  7. On the left, click Pop-up Windows.
  8. Make sure Allow is chosen in the pop-up menu next to the website.

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until December 31, 2024. The initial cut-off date for referral of eligible applications will be 10 days after the JOA is posted, with subsequent referrals will be contingent on the availability of vacant positions. Eligible applications received after that date will be referred at regular intervals OR as additional vacancies occur on an as-needed basis until positions are filled.

Duties:

  • Assesses, plans, implements, and evaluates care based on age-specific components.
  • Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self -management after discharge, and supporting factors that impact customer satisfaction.
  • Considers all characteristics of the individual, including age and life stages, state of health, race, culture. values, and previous experiences.
  • Administers medications and procedures per established policies and guidelines.
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
Preferred Experience:
  • Medical surgical experience,
  • Geriatric Outpatient RN Care Manager experience preferred
