• Collaborate with primary care providers, clinic nurses, public health nursing, DM care management, social services, and contracted vendors to ensure that the patient's care needs are completed in a timely and cost effective manner to maximize each visit.
• Responsible to manage patient care coordination in delivery of PRC service; will review, determine, and coordinate PRC care for assigned patients with other members of the multidisciplinary team in order to optimize patient care.
• Conducts planning for scheduled patients and communicates the patient needs and recommended treatment to care team members.
• Participates in departmental orientation, on the job training and quality improvement programs/initiatives; documents care appropriately in the electronic health record; and routinely assess client's status and program.
• Manages aspects of the planning progress in anticipation of patient variances and transitions collaborate and communicates with the physician and the rest of the care management multidisciplinary team to facilitate healthcare planning.
