The LPN may contribute to the plan of care by collaborating with interdisciplinary team members, the patient and the patient's family. The LPN collects data and information, recognizes changes in conditions and reports this to the RN supervisor or another appropriate clinical supervisor to assist in the identification of problems and formulation of goals, outcomes, and patient-centered plans of care that are developed in collaboration with patients, their families, and the interdisciplinary health care team. The LPN participates in the nursing process by appraising the individual patients status or situation at hand. The LPN can participate in implementing established teaching plans for patients and their families with common health problems and well-defined health learning needs and suggest any modifications to the plan of care that may be necessary. The PACT LPN Clinical Associate facilitates continuity of care by making the appropriate referrals. Adhoc telework available at supervisor discretion.
Demonstrates competence in delivering and improving holistic care through collaborative strategies with others. The GS-7 LPN has an exceptional degree of knowledge and skills necessary for self-direction in carrying out the most complex practical nursing care assignments for both inpatient and outpatient care. Assignments include performing nonstandard and specialized activities sufficient to solve a range of complex nursing problems of considerable difficulty and responsibility, under very general supervision. In addition to meeting the requirements for the LPN GS 3-6 levels, possess broader knowledge and skills for increased involvement in the development and modification of the Veterans plan of care, ability to exercise independent technical judgment, and possess and apply a comprehensive working knowledge of the principles, art, and science of practical nursing. Positions at this grade level require broader knowledge and skills because supervision is more general, and employees are expected to be involved to a greater degree in the development and modification of treatment plans.
Work Schedule: 7:30am - 4:00pm (Monday - Friday), flexible to meet needs of the organization
