The HCBS Wait List Care Coordinator is responsible for establishing a set of person-centered goal-oriented, culturally relevant, and logical steps to ensure that a person receives services in a supportive, effective, efficient, timely and cost-effective manner. Care coordination includes case management, disease management, discharge planning, transition planning, and addressing social determinants of health and integration into community.
Working Schedule: Schedule is Monday through Friday between the hours of 8 am to 5 pm. This position is a field-based position with a home-based office. You will work from home when not in the field.
Location: Reno, Butler, & Sedgwick counties and surrounding areas
Local travel up to 75% and mileage is reimbursed at current government rate.

Primary Responsibilities
- Assess, plan, and implement care strategies that are individualized by the individual and directed toward the most appropriate, least restrictive level of care
- Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
- Ensure the care plan/plan of service includes covered services, EPSDT, in lieu of services, value-added benefit, informal supports, and addresses social determinants of health
- Communicate with all stakeholders the required health - related information to ensure quality coordinated care and services are provided expeditiously to all persons
- Advocate for persons and families as needed to ensure the persons needs and choices are fully represented and supported by the health care team
- Conduct home visits in coordination with person and care team, which may include a community service coordinator
- Conduct in-person visits which may include nursing homes, assisted living, hospital, or home
- Serve as a resource for community care coordinator, if applicable

Required Qualifications
- Bachelor’s Degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area
- 1+ years of experience working with persons with long-term care needs
- 1+ years of experience working within the community health setting in a health care role
- Intermediate level of computer proficiency including the use of Microsoft Outlook, Teams, and multiple web applications
- Ability to travel 75% of time for role
- Valid driver's license, current automobile insurance and access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information and live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
- Licensed Social Worker or clinical degree
- At least one year experience working with persons receiving services on one of the home and community based waivers in KS
- Background in managing populations with complex medical or behavioral needs
- Experience with electronic charting
- Experience with arranging community resources
- Strong communication and customer service skills both in person and via phone
- Ability to work independently and maintain good judgment and accountability
- Demonstrated ability to work well with others
- Strong organizational and time management skills
- Ability to multi-task and prioritize tasks to meet all deadlines
- Ability to work well under pressure in a fast-paced environment
- Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others
