We are seeking a dedicated Hospice Start of Care Hospice Registered Nurse (RN) to join our growing team. In this role, you will play a critical part in assessing and initiating care for patients newly referred to hospice. You will work closely with interdisciplinary teams, patients, and their families to ensure the highest standard of care and comfort for each individual.

We are seeking a dedicated Hospice Start of Care Hospice Registered Nurse (RN) to join our growing team. In this role, you will play a critical part in assessing and initiating care for patients newly referred to hospice. You will work closely with interdisciplinary teams, patients, and their families to ensure the highest standard of care and comfort for each individual.
Our Hospice SOC Registered Nurse will enjoy some notable benefits:
- Salary: $55.00 - $60.00 per hour with a sign-on bonus up to $15,000 distributed over two years
- Available position: Wednesday-Sunday, must have availability for admissions between 7 AM and 7 PM

- Conduct the admissions process in a timely, patient-centered manner, ensuring excellent customer service.
- Complete efficient admissions that meet both external and internal customer needs, including accurate and prompt documentation.
- Develop an initial plan of care that addresses the patient’s needs at the time of admission, in collaboration with the interdisciplinary team (IDT).
- Provide specialized hospice nursing services, educating and supporting patients and families on care needs, including pain and symptom management.
- Coordinate patient care and ensure smooth transitions from SNF, board and care, or home by sharing relevant admission information.
- Collaborate with Team Leaders and supervisors to ensure effective communication and teamwork across disciplines, including educating healthcare providers and referral sources as needed.
- Graduate of an accredited nursing program and active RN license in California
- Minimum one year of hospice experience. Previous experience in Start of Care assessments and planning is a plus
- Ability to work independently, while collaborating with a multidisciplinary team
- Strong clinical skills and attention to detail
- Compassionate, empathetic, and patient-focused approach to care
- Excellent communication and organizational skills
- Current CPR certification and valid driver’s license with reliable transportation required
