
Remote Case Manager RN
The Nurse Case Manager II (NCM) is responsible for comprehensive patient case management, including coordination for discharge planning, transition of care needs, and outpatient management. The role involves identifying, tracking, and monitoring patients with multiple co-morbidities and/or psychosocial needs, developing action and discharge plans, and ensuring the continuity of care. The NCM collaborates with hospitals, physician teams, and care teams to provide appropriate levels of care and transition planning. This role also includes providing health education, coaching, and treatment decision support for patients. The NCM acts as an advocate for patients and their families and works with Care Advocates and Social Workers.
Essential Job Functions:
- Engage patients and caregivers telephonically to establish and assess action plans.
- Provide member education for self-management goals and contingency plans.
- Identify patient needs, close healthcare gaps, and develop action plans.
- Develop interventions based on evidence-based practice.
- Ensure discharged patients receive necessary services and resources.
- Conduct transition discharge assessments to identify member needs.
- Serve as clinical liaison and perform clinical authorizations for inpatient care.
- Make referrals to community sources and programs.
- Use motivational interviewing techniques to gather health history and determine health literacy.
- Manage treatment plan assessments and collaborate with healthcare providers.
- Collaborate with the Interdisciplinary Care Team for individualized transition plans.
- Confer with Medical Directors regarding inpatient cases and participate in huddles.
- Demonstrate knowledge of utilization management and transition planning.
- Maintain knowledge of company products and customer needs.
- Manage caseload efficiently using time management skills.
- Document care management activities accurately.
- Maintain licensure and hospital credentialing.
- Perform other related duties as assigned.
Must haves:
1+ years of Case Management Exp
Active Compact RN License
Compensation:
$30.00/hr to $39.00/hr.
Exact compensation may vary based on several factors, including skills, experience, and education.
