Job Overview – Healthcare Support Consultant (Hybrid):

Step into a key role as a Healthcare Support Consultant with our client in Saint Paul, MN. In this position, you will educate participants in the Community First Services and Supports program, assist in crafting service delivery plans, conduct quality reviews, and provide recommendations to enhance service delivery. This hybrid role combines remote work with in-home visits to participants, requiring strong communication, organization, and case management skills.

Compensation: $50,000 - $55,000/year

Location: Saint Paul, MN

Schedule: Monday to Friday (Hybrid)

Responsibilities as the Healthcare Support Consultant:

  • Participant Education: Meet with individuals in their homes or remotely to provide guidance on program models, rules, processes, and policies.
  • Service Planning: Assist in creating service delivery and quality review plans tailored to participant needs.
  • Home Visits: Conduct in-home visits to diverse participants, managing a caseload efficiently and ensuring timely delivery of services.
  • Quality Review: Evaluate service utilization and provide actionable recommendations for improvement.
  • Documentation and Reporting: Maintain timely and accurate documentation of all interactions and plans, completing visit notes during or immediately after each visit.
  • Travel Management: Organize and prioritize home visits to ensure efficient use of driving time, with the ability to travel over 60 miles as needed.

Qualifications for the Healthcare Support Consultant:

  • Education: Bachelor’s degree in social work, sociology, psychology, or a related field.
  • Experience: 2-3 years of case management with vulnerable populations in home settings, and 1-2 years in social services or healthcare, with knowledge of Medicare and Medicaid.
  • Technical Skills: Proficient in Microsoft Office and electronic records systems.
  • Attributes: Strong verbal and written communication skills, excellent organizational abilities, and the capacity to prioritize tasks in a dynamic environment.
  • Licensure and Transportation: Valid driver’s license, satisfactory driving record, and reliable, insured vehicle required.
  • Teamwork: Ability to function effectively in a team setting, collaborating professionally with colleagues during meetings and fieldwork.
