  6. Make sure Off is chosen in the pop-up menu next to the website.
  7. On the left, click Pop-up Windows.
  8. Make sure Allow is chosen in the pop-up menu next to the website.

Requirements

As a Home Health Case Manager RN, you’ll provide skilled nursing services such as infusion therapy, wound care, ostomy teaching, and disease management—all in the comfort of patients’ homes. With our comprehensive orientation and ongoing educational support, you’ll have the tools to excel and grow in your career.

  • Schedule: Monday - Friday, 8 AM - 5 PM
  • Preferred Experience: Home health, infusion, and 2-3 years of diverse nursing experience (willing to train the right candidate).
  • Required Qualifications:
    • Current driver’s license and car insurance
    • CPR BLS & First Aid certifications
    • Negative TB test (provided)
    • Arizona Level One Fingerprint Clearance Card
Responsibilities
  • Develop personalized care plans in collaboration with physicians.
  • Deliver hands-on nursing care and provide education to patients and families.
  • Schedule visits and coordinate care with the clinical team.
  • Administer IV therapies, manage central lines, and draw labs while adhering to infection control practices.
  • Complete documentation promptly using our advanced EMR systems.
