Jet Health, Inc / First Choice Home Health is seeking a dedicated full-time home health Clinical Scheduler to join our home health team. The primary responsibility of this role is to efficiently schedule patient/client services and coordinate with staff to ensure the delivery of high-quality care to our patients in Boise, ID, and surrounding areas.
This position could potentially handle a scheduling case load of 200+ patients.

Requirements
- Education: High School graduate.
- Experience: One year's experience as a home health scheduler highly preferred.
- Skills: Exceptional communication skills, experience with electronic medical records (HCHB preferred), ability to multitask in a fast-paced environment.
- Home Care Home Base experience highly preferred but not required
- Strong MS Office experience, Excel & Word required
- Willingness to help in other office functions as needed
- Team player with a can-do attitude
- Strong attention to detail
- Ability to follow direction and retain information
- Evaluation
- Schedules and appropriately documents patient/client schedules in a timely manner.
- Participates in coordinating care with management and patient/client interdisciplinary team.
- Provides effective communication to patients/clients, staff members, other health care professionals, and referral sources.
- Processes reports timely to ensure compliance.
- Responds to Agency and patient/client needs in a professional and creative manner.
- Promotes the agency philosophy and mission by presenting a positive image to patients, families, physicians, and community agencies.
- Demonstrates commitment, professional growth, and competency.
- Provides effective communication to patient/family, team members, and other health care professionals and maintains confidentiality.
