Job Description

The Internal Care Coordinator (ICC) plays a crucial role in collaborating with Clinical Outreach Representatives, Facility Leadership, and Treatment Team members to ensure patients receive appropriate referral options upon discharge. This position serves as a liaison between facility staff and outreach representatives to facilitate seamless aftercare planning and ensure continuity of care.

Duties & Responsibilities

  • Act as a key point of contact between facility staff, clinical outreach, and referring providers to support patient aftercare coordination.
  • Ensure accurate documentation and tracking of aftercare plans within EMR and CRM systems.
  • Support treatment teams in identifying and coordinating appropriate aftercare referrals.
  • Participate in facility meetings and provide training sessions to new team members.
  • Collaborate with internal departments to address patient needs and improve processes.
  • Maintain confidentiality and compliance with all organizational policies and procedures.
