Position Overview

The Pride Center of Maryland (PCOM) is seeking a dedicated Linkage to Care Specialist to facilitate access to essential health and social services for LGBTQ/SGL(same gender loving) individuals, particularly those affected by sexual health and risk reduction concerns and other health disparities. This role involves connecting clients to medical care, mental health services, and community resources while providing compassionate support and education. The ideal candidate is passionate about health equity, possesses strong case management skills, and thrives in a community-centered environment.

Key Responsibilities

  • Client Engagement & Assessment: Conduct comprehensive needs assessments for clients seeking health and social services, ensuring culturally competent and affirming support.
  • Service Coordination: Facilitate timely linkage to medical care, mental health services, housing assistance, substance use treatment, and other critical resources.
  • Navigation & Advocacy: Assist clients in overcoming barriers to care, including transportation, financial constraints, and healthcare stigma, by providing referrals and direct advocacy.
  • Education & Counseling: Provide health education, risk reduction counseling, and adherence support for individuals managing chronic health conditions, including HIV/AIDS.
  • Collaboration & Networking: Work closely with healthcare providers, case managers, and community organizations to strengthen PCOM’s network of support services.
  • Data Management & Reporting: Maintain accurate client records, document service linkages, and contribute to program evaluation and reporting efforts.
  • Outreach & Community Engagement: Participate in outreach events, health fairs, and workshops to increase awareness of PCOM’s services and promote health equity.

Qualifications & Experience

  • Education: Bachelor’s degree in social work, public health, psychology, or a related field preferred; relevant experience may be considered in lieu of a degree.
  • Experience: Minimum of 2 years of experience in case management, social services, healthcare navigation, or community outreach, preferably working with LGBTQ/SGL and/or marginalized communities.
  • Knowledge: Familiarity with HIV/AIDS care and sexual health, PrEP navigation, social determinants of health, and community-based health resources.
  • Skills: Strong interpersonal, communication, and organizational skills with the ability to engage diverse populations in a compassionate and nonjudgmental manner.
  • Technology: Proficiency in Microsoft Office, electronic health records (EHR), and data entry systems.
  • Language: Bilingual (English/Spanish) is a plus but not required.
