Essential Functions And Principal Accountabilities
- Identify high-risk and complex populations within Government Programs and develop and implement programs to increase access and focus on medical-dental integration.
- Lead care coordination and outreach activities for Government Program high risk or complex oral health needs members.
- Lead care coordination efforts for internal escalated members and complete oral health survey as needed.
- Lead care coordination efforts for identified cases resulting from appeal, grievance, or State Fair Hearing resolution.
- Lead implementation and monitoring of PIP and quality improvement projects for high-risk or complex oral health needs populations to meet contract and internal goals.
- Lead ER data project by coordinating care for members, relationships with MCOs, and identify education and outreach materials with emergency rooms within the State.
- Lead all mobile dental clinics including site logistics, relationship with mobile dental vendor, and scheduling for members. Lead follow up care coordination efforts as needed.
- Lead and monitor single case agreements to ensure member access which includes collaboration with internal clinical team, providers, and members. Complete necessary quarterly reporting as required.
- Develop resources that educate members on the Government Programs benefits and oral/overall health practices.
- Provide support for the external quality review audits.
- Provide support, input, and review for requests for proposals and other bids as necessary for Government Programs business.
- Represent Delta Dental Government Programs at various outreach events, coalitions, and conferences in addition to HHS Member and Provider Town Halls.
- Build relationships with individuals and organizations to facilitate coordination with community-based resources, as well as medical and dental providers.
- Work with community-based services and representatives to educate and assist program members about behaviors that can enhance their oral health, as well as their overall health, and successfully navigate program services.
- Work with leadership and other internal stakeholders to develop and distribute internal and external communications for the program with all outlets (social media, print and television news; stakeholders, etc.).
- Provide leadership to educate customer service representatives on effective communication with members.
- Coordinate with Delta Dental foundation staff related to Iowa oral initiatives and efforts.
- Collaborate with data analysis staff to develop and maintain a report system for outcomes.
- Collaborate with Member and Provider Services staff to communicate Government Program member issues requiring interventions to appropriate internal departments and providers.
- Maintains confidentiality and uses only the minimum amount of protected health information (PHI) necessary to accomplish job related responsibilities.
- Perform other duties as assigned.
Required
- Bachelor’s degree in health education or public health, public administration, social work or a related field of interest required.
- Five years’ experience in a nonprofit/public health or social work position.
- Experience with collaboration, coalition building and project management skills.
- Excellent verbal and written communication and interpersonal skills.
- Basic knowledge of Iowa’s human service and public health agencies.
- Time management skills and flexibility to perform well under time pressure and with shifting priorities.
- Ability to translate and follow contract language, federal regulations, and policies.
- Proficient in use of Microsoft Office applications (Word, Excel, PowerPoint).
- Ability to work independently.
- Ability to travel is required in state 20%.
- Ability to work flexible hours, including evenings and weekends, is required.
- Must possess a valid State of Iowa driver’s license.
- Master’s degree with emphasis in social work or public health preferred.
- Licensed to perform social work in the State of Iowa preferred.
- Dental and/or insurance knowledge preferred.
- Bilingual candidates (English and Spanish) preferred.
