To assist individuals who have recently been evaluated for being in emotional and behavioral crisis, either requiring involuntary or voluntary hospitalization in a Crisis Stabilization unit or safety planning and immediate resource brokering by Rapid Responder, in gaining information and access to needed medical, social, educational, vocational, and other services. The primary goal is to assist clients in accessing further treatment. This position can be completed remotely.

To assist individuals who have recently been evaluated for being in emotional and behavioral crisis, either requiring involuntary or voluntary hospitalization in a Crisis Stabilization unit or safety planning and immediate resource brokering by Rapid Responder, in gaining information and access to needed medical, social, educational, vocational, and other services. The primary goal is to assist clients in accessing further treatment. This position can be completed remotely.
- Utilizes a short-term case management model to provide resources, brokering and education for a minimum of three days following the crisis assessment.
- Follows up and links the client with specified services and resources as identified by the Rapid Responder or determined by the case manager.
- Maintains regular contact with Rapid Responder to coordinate appropriate level services and collaborate.
Education and Experience Requirements for Mobile Response Case Manager:
- Bachelor’s degree in a social services field is required.
- A minimum of one year of experience working with clients in the behavioral health field.
- Good computer skills and ability to adapt to electronic health record changes.
- Previous experience working in a mental health crisis response environment.
