  • Must possess a working knowledge & experience in use of medical & mental health diagnoses, disabilities, & treatment procedures, including acute, chronic & traumatic illnesses, common medications & their effects/side effects, & medical terminology.
  • Must facilitate discharge planning through collaboration with Veterans & their families as well as interdisciplinary treatment team members to ensure that appropriate discharge plans are executed in a timely manner.
  • Must provide consultation &education to Veterans &their families regarding community resources, VA benefits &specialty programs, & advance directives.
  • Must maintain knowledge of Veteran's benefits & services, community resources, &process for making appropriate referrals to community & other governmental programs or agencies.
  • Must possess skills &knowledge to provide advanced case management services to Veterans & their families throughout the continuum of care.
  • Is responsible for supporting the mission, policies, & procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), &the facility.
  • Is willing to serve on committees, work groups, &task forces at the facility as deemed appropriate by the supervisor &Chief of Social Work Services.
  • Must keep their supervisor apprised of problems &recommended solutions to problems encountered in the incumbent's area of responsibility.
  • Must supervise, as appropriate, the practice of social work students or a less experienced social worker.
  • Must demonstrate knowledge &skill in the use of software applications for drafting documents &data management as well as other computer systems in use by the VHA.
  • Must maintain a level of productivity & quality consistent with the complexity of the assignment &consistent with Social Work standards, accrediting body standards & facility standards.
  • Will document all encounters for appropriate workload capture in accordance with ECHCS policies & professional Social Work standards.
  • Will participate in a modification of tour of duty to accommodate patient care needs & coverage on weekends.
  • Will need to drive a government vehicle to travel throughout the Denver metro area.
  • Must possess &maintain a current valid state driver's license.

  • Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to the social worker position GS-0185 series in the VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to .

Loss of Licensure or Certification. Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determination Requirements: Candidates must possess the following grade determination requirements in addition to basic requirements

Social Worker-Program Coordinator, GS-12

Experience/Education.
One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.

AND

Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.

AND

Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all the following KSAs:

1) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
2) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
3) Ability to supervise multidisciplinary staff assigned to the program.
4) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
5) Ability to provide training, orientation, and guidance within clinical practice.

Some travel is required as the incumbent is required to conducted outreach to community partners and provide community based case management to Veteran residents which may require traveling long distances. The emotional demands can be stressful in working with high-risk for suicide veteran population and their families. A high level of self-awareness is essential. Work, divided between office and clinical settings, is primarily sedentary. May require prolonged standing, moderate walking, and manual dexterity.
