Requirements

  • Must have a New York State RN License
  • Must have prior experience in patient care setting
  • Must have experience in a home care agency, and strong knowledge of regulations
  • Demonstrates knowledge of nursing practices, techniques and methods applied to health and wellness resident care, and federal, state, and local regulations
  • Demonstrates knowledge of good assessment skills
  • Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care

Responsibilities Inlcude

  • Identifying the continuous needs and services for patients, working to deliver the highest standards of services
  • Ensuring appropriate follow-up, assessment, and documentation for any changes in the healthcare services and status of patients
  • Conducting wellness visits
  • Takes the lead in maintaining regular communication with the patients' family or responsible party, contacting the patients' attending physician when necessary
  • Responsible for overseeing the general medical health of the patients, ensuring monthly checks for weights, vital signs, etc
  • In addition, they assist in maintaining wellness files in accordance with company policies and state and federal regulations, and are responsible for the maintenance of medical supplies and emergency kits
