- Serves as a Nurse Case Manager in a multidisciplinary team to provide assessment, planning, implantation, coordination, evaluation and monitoring of patents for health options and services.
- Provides advanced practice clinical nursing, administrative and organizational skills in managing the continuity of care for the beneficiary populations.
- Conducts comprehensive assessments of patients health needs in order to develop a plan of care.
