Nashville General Hospital is hiring a Case Manager for our Emergency Department for full-time Days!

The RN Case Manager of the Emergency Department (ED) is responsible for establishing, coordinating, and maintaining the process to increase patient throughput to the most appropriate level of care while facilitating interdisciplinary care across the continuum of the ED. The RN Case Manager collaborates with the patient/family, multidisciplinary team, physicians, and the Community Care Team (CCT). The scope of practice includes patient/family assessment and management, care facilitation, care transitions, discharge planning with referral to all levels of care, and other related duties specific to the defined patient population. This position is primarily responsible for providing clinically based case/disease management to support the delivery of effective and efficient patient care in the inpatient and outpatient, with a specialty in the Emergency Department. This is a Monday-Friday position working 11am-7pm and a rotating holiday/weekend coverage.

As a Case Manager, you will:

  • Assess patients' physical, functional, social, psychological, environmental, learning, and financial needs.
  • Identify problems, goals, and interventions designed to meet the patient’s needs, including prioritized goals that consider the patient/caregiver's goals, preferences, and desired level of involvement in the case management plan.
  • Assess the patient’s formal and informal support systems, including caregiver resources and involvement, as well as available benefits and/or community resources.
  • Provide referrals to appropriate community resources, including the Community Care team (CCT); facilitate access and communication when multiple services are involved; monitor activities to ensure that services are being delivered and meet the needs of the patient; coordinate services to avoid duplication.
  • Identify risk factors and teach patients a clear pathway of response to identified triggers.

Required Skills & Qualifications:

  • Must be currently licensed in the State of Tennessee as a Registered Nurse
  • An Associate Degree or Diploma in Nursing is required; Bachelor’s Degree in Nursing or a related healthcare field is preferred
  • Must possess a minimum of one year of experience in clinical setting
