Essential Duties, Functions And Responsibilities

  • Maintains a system of tracking referred patients for admission in the other health care facilities and performs chart reviews to determine timeliness of requested services to facilitate the return of patients, or other health care facilities or discharge to home. Reports to providers and discusses appropriateness of diagnostic and treatment procedures and the best placement of each patient considering the diagnoses, prognoses, and services available.
  • Assists patients in understanding, accepting and adjusting to complication of diagnoses and treatment, and accepting responsibility for medical care and treatment.
  • Collects relevant data for program evaluation regarding the discharge planning process, decreasing the re-admission rate.
  • Establishes discharge evaluation upon admission and maintain discharge planning process throughout inpatient stay. Provides discharge planning assessment for each inpatient.
  • Supervises/develops discharge evaluation-need for post discharge services, patient capacity for self-care, safety, special equipment needs, post discharge transport, and wound care.
  • Discusses discharge plan with each patient/family, document discussion, and who participated in the care plan.
  • Includes discharge evaluations in each patient’s medical record.
  • Reviews patient medical records to determine that established criteria are met. Identifies patient at high risk needing community services.
  • Identifies patients needing referrals, follows up on referral(s) and documents in the patients’ medical record.
  • Develops and supervises discharge planning evaluations and ongoing evaluations.
  • Establishes and maintains the multi-disciplinary discharge planning committee daily, minutes and follow up on plans for each patient.
  • Documents in the patient medical record care issues identified and addressed interventions, recommendations and/or referrals being made.
  • Rounds daily on assigned patients with the patient care team.
  • Works clinically on the assigned units providing direct patient care.
  • Completes all yearly Center for Medicare and Medicaid Services (CMS) required training by the indicated due dates.
  • Assists, promotes, and supports compliance with established CMS standards.
  • Performs other duties as assigned.

Mandatory Minimum Qualifications

Experience:

Two (2) years of Registered Nurse experience in a clinical, medical/surgical, acute and/or critical care setting.

Education

Associate of Science degree in Nursing from an accredited nursing program.

License

Valid and unrestricted Registered Nurse license in any U.S. State or Territory.

Certificate

Valid American Heart Association (AHA) certification in Basic Life Support (BLS) and maintain certifications.
