Position Overview: The Behavioral Health Care Manager (BHCM) plays a pivotal role in the implementation and management of the Collaborative Care Model (CoCM) within a primary care setting. The BHCM works closely with primary care providers (PCPs), psychiatric consultants, and other healthcare team members to deliver evidence-based behavioral health interventions. The primary goal is to enhance the mental health outcomes of patients by providing effective, patient-centered care coordination, assessment, and intervention.

Key Responsibilities:

Care Coordination & Case Management:

  • Coordinate care and facilitate communication between patients, primary care providers, psychiatric consultants, and other healthcare team members.
  • Develop and implement individualized care plans in collaboration with the patient and the healthcare team.
  • Monitor patient progress and adjust treatment plans based on clinical guidelines and patient outcomes.


Patient Engagement & Support:

  • Conduct initial behavioral health assessments, including standardized screening tools for depression, anxiety, and other mental health conditions.
  • Provide brief, evidence-based therapeutic interventions (e.g., problem-solving therapy, motivational interviewing).
  • Educate patients on their diagnosis, treatment options, and self-management strategies.
  • Offer proactive follow-up and ongoing monitoring of symptoms, treatment adherence, and patient satisfaction.


Collaboration & Consultation:

  • Work closely with the psychiatric consultant to review cases, refine treatment plans, and adjust interventions as needed.
  • Communicate regularly with the primary care team to ensure integration of behavioral health care within the patient’s overall treatment plan.
  • Participate in team meetings, case reviews, and quality improvement initiatives.


Data Management & Documentation:

  • Maintain accurate and up-to-date patient records, including assessment notes, care plans, progress updates, and treatment outcomes.
  • Utilize the electronic health record (EHR) to document all patient interactions and clinical decision-making.
  • Track and report on key metrics related to patient outcomes, care management activities, and program performance.


Professional Development & Compliance:

  • Stay current with best practices and evidence-based guidelines in behavioral health and integrated care.
  • Participate in ongoing training and development opportunities.
  • Adhere to all legal, ethical, and organizational policies regarding patient care, confidentiality, and documentation.


Qualifications:

  • Education: Degree in social work (MSW), counseling, psychology, or a related field.
  • Licensure: Licensure as a social worker (LCSW, LICSW), professional counselor (LPC), or equivalent in the state of practice.
  • Experience: Minimum of 2 years of experience in behavioral health care, preferably within a primary care or integrated care setting.
  • Skills:
  • Strong knowledge of behavioral health assessment and intervention strategies.
  • Proficient in care coordination and case management.
  • Excellent communication and collaboration skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Familiarity with electronic health records and data management systems.


Preferred Qualifications:

  • Experience with the Collaborative Care Model (CoCM) or other integrated care frameworks.
  • Certification in relevant therapeutic interventions (e.g., CBT, DBT, PST).
  • Bilingual/multilingual capabilities
