Job Description

Serengeti Care is seeking a Client Care Manager. The Client Care Manager operates under the direction of the Regional Manager, Director of Business Development and General Manager to oversee all aspects of care delivery and may work in collaboration with the RN Case Manager on clinical matters. The Client Care Manager works directly with caregivers.

Responsibilities

  • Calls on physicians, hospitals, skilled nursing, and rehabilitation facility discharge planning services and/or management within a given territory,
  • Responsible for new business generation/growth through Marketing and Community connections,
  • Oversight of Caregiver hiring and Manage Client Care scheduling activities,
  • Visit Clients within assigned area regularly,
  • Meet with potential Clients and family members to discuss their needs and provide solutions in the form of a Plan of Care,
  • Maintain ongoing communications and a consistent follow-up schedule with Potential Clients and Families who have yet to commit to service,
  • Work with Team Members to coordinate various aspects of Client's care,
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of Clients through a Quality Assurance Program,
  • Supervises all Caregivers in the Region.

Qualifications

  • College Degree Preferred but Not Required, Healthcare Credential Preferred
  • Three Years of Experience in In-Home Care, Healthcare or Care-Related Industry Required, Hands on Caregiving experience preferred,
  • An equivalent combination of education and work experience will be considered,
  • Must possess a Valid Driver's License, have reliable transportation and automobile insurance.

Knowledge, Skills, And Abilities

  • Minimum of one year of experience in an In-Home Care/Home Health setting in a Supervisory Role.
  • Experience with Medicaid/DSHS and Veterans Services is a Plus+
  • A general understanding of compliance and state regulations with regards to In-Home Care,
  • Experience should emphasize problem-solving skills in a Client Care setting.
  • Demonstrate Excellent Oral and Written communication skills and the ability to listen and comprehend effectively.
  • The ability to work independently, maintain the confidentiality of information and meet scheduled deadlines.
  • Demonstrate effective Interpersonal Skills as well as sound judgment and above-average decision-making skills.
  • Demonstrate discretion, integrity, and honesty consistent with office standards, practices, policies, and procedures.
  • Must demonstrate knowledge of the Senior Care industry.
  • The ability to organize and prioritize Daily, Monthly, Quarterly and Yearly Work,
  • The ability to establish good working relationships with Care for all Staff.
  • The ability to present a Professional Appearance and Demeanor.
  • The availability to work Evenings or Weekends as Required.
  • The ability to perform duties in a Professional office setting.
  • The ability to work as part of a Team while focusing on employee retention.

Pay range: $65,000-$75,000 Annually
