Job Description
Helping with UR transmission. Coordinates and implements the transition of care (TOC) / Discharge (DC) plan for ambulatory patients. Prioritizes and coordinates the plan across the care continuum through critical thinking, teamwork, and communication between care providers, patients, families, and external vendors to ensure timely discharge.

Hard Skills
- Data Entry Experience
- Knowledge of payers
- Auth/case management experience preferred
- EPIC experience
- Ability to type 55 or more WPM
- Good Communication skills
- Teamwork skills

- Adaptable to change
