EDUCATIONAL QUALIFICATIONS:

High School diploma or equivalent. / Case Manager Qualifications include a Bachelor’s degree from an accredited college in rehabilitation, counseling, psychology, sociology, social work, or a related discipline (a combination of equivalent education and work experience may be considered).

EXPERIENCE QUALIFICATIONS:

  • Minimum of One year’s experience in a rehabilitation, non-profit facility, or comparable setting in the area(s) of case management, service provision, working with individuals with various disabilities, job development, job placement, follow-up, and training
  • Proficiency with Microsoft Office suite and general computer operation is required
  • Experience in marketing and public relations.
  • Strong working knowledge of disability management and awareness

Case Manager Qualifications

  • Three or more years experience in a rehabilitation, non-profit facility, or comparable setting in the area(s) of vocational rehabilitation, non-profit facility, substance abuse, mental health counseling, service delivery, or case management
  • Proficiency with Microsoft Office suite and general computer operation is required
  • Working knowledge of basic case management principles
  • Strong working knowledge of disability management and awareness
  • Additional Preferred Qualifications: Broad knowledge base of disabilities and issues influencing those with various conditions
  • Professional Certification of or equivalent certification preferred in: N/A

JOB RESPONSIBILITIES SUMMARY:

Ability to job develop and serve as an advocate for individuals with disabilities eager to obtain employment. Assist referred individuals with job placement, follow up and training. Provide case management and social service referral assistance as needed. Provide job readiness training and instruction.

Case Manager's Additional Responsibilities

Provide vocational services to individuals with disabilities ensuring they remain gainfully employed. Must be timely and detail-oriented about case management requirements. Maintain communication with referral agencies, family members, workplace supervisors, and other involved parties. Remain compliant with AbilityOne and CARF regulations and adhere to company policies related to service provision.

ESSENTIAL FUNCTIONS:

  • Complete training as defined by the Phoenix Professional Development Plan
  • Provide information as requested for various statistical requirements
  • Maintain reporting and records as outlined by policy, referral source, and accreditation requirements
  • Know current contract requirements, AbilityOne regulations, CARF standards, social security guidelines, Phoenix mission/policies, departmental goals and objectives and other sponsoring agency guidelines

Case Manager's Additional Functions

  • Have a working knowledge of current contract and referral source requirements as well as CARF standards, DOL, social security guidelines, and Phoenix mission and internal policies
  • As necessary develop, write, and present progress reports and discharge summaries documenting performed activities
  • Complete training as defined by the Phoenix Professional Development Plan
  • Represent Phoenix in the business community by involvement in civic and professional organizations
  • Provide information as requested for various statistical requirements

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

  • Adequate ability for conversation, telephone use, and emergency information systems
