Major duties include, but are not limited to the following:
- Able to anticipate and implement preventive patient care measures. This PACT is responsible for direct patient care on the PACT teams and facilitating the appropriate plan of care.
- Expected to manage patient care in an astute manner, which promotes positive patient outcomes including the areas of
- triage, telephone triage, and scheduled and unscheduled patients.
- Possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs, providing care as considered necessary.
- Provides guidance to all caregivers and collaborates with the PACT Teamlet regarding the plan of care and educational requirements as indicated to
- support patient needs.
- Reports status of service needs and contributes to problem resolution and modification of processes as necessary.
- Other duties as assigned.
Pay: Competitive salary, regular salary increases, potential for performance awards
