Medical Records Manager

Job Summary: The Medical Records Manager is responsible for overseeing and managing the medical records department. This role involves ensuring that patient medical records are accurate, complete, and compliant with federal, state, and local regulations. The ideal candidate will work closely with the home healthcare team to facilitate proper documentation and maintain confidentiality and security of patient information.

Responsibilities:

Records Management:

  • Oversee the collection, storage, and retrieval of patient medical records.
  • Ensure that records are maintained in compliance with HIPAA and other relevant regulations.
  • Implement and maintain procedures for Homecare Homebase (HCHB).
  • Conduct regular audits of medical records to ensure accuracy and completeness.


Compliance and Quality Assurance:

  • Ensure adherence to all federal, state, and local regulations related to medical records.
  • Develop and enforce policies and procedures for medical record management.
  • Provide training to staff on proper documentation practices and compliance requirements.
  • Address and resolve any issues related to medical record discrepancies or compliance violations.


Data Management:

  • Oversee the input and updating of patient information in HCHB.
  • Generate and review reports to monitor the quality and integrity of patient records.
  • Ensure timely and accurate data entry and retrieval for patient care and billing purposes.


Collaboration and Communication:

  • Work closely with healthcare providers to ensure proper documentation of patient care.
  • Communicate with patients, families, and external agencies regarding medical record requests and releases.


Confidentiality and Security:

  • Maintain the confidentiality and security of all patient information.
  • Implement and enforce measures to protect patient records from unauthorized access or breaches.


Administrative Duties:

  • Supervise and train medical records staff as needed.
  • Prepare and present reports on medical records management activities to senior management.


Qualifications:

  • Two (2) years of experience in orders/records management or a similar role; preferably within a home health or hospice setting.
  • Two (2) years of experience in HCHB EMR.
  • Strong understanding of healthcare regulations, including HIPAA.
  • Excellent organizational, analytical, and problem-solving skills.
  • Strong communication and interpersonal skills.
  • Ability to manage multiple tasks and prioritize effectively.
